Dental Products Report - February 2011 - (Page 92) CLINICAL 360° TECHNIQUE IN PRACTICE USE SONIC INSTRUMENTS Preparing tooth structure for porcelain laminate veneers. by D R S . MARIO ROTELLA AND ALESSANDRO GEMINIANI 1 HOW TO Information provided by KOMET USA. The esthetic demands of patients have substantially expanded during the last 20 years. The controversy regarding treatment planning criteria for a patient seeking esthetics still remains. Arguments exist for both aggressive and conservative treatment approaches. Long-term prognosis plays a role in the decision of the type of treatment provided to the patient. The overall survival rate of metal-ceramic fi xed dental prostheses (FDP) ranges from 61.5% to 73.1% after 20 years.1-6 Porcelain laminate veneers (PLV) were introduced by Pincus in 1930 and have become a popular dental procedure.7 The advantage of PLV is the extremely conservative tooth preparation,8 thus preserving as much tooth structure as possible. The survival rate of PLV is high, and several studies report survival rates more than 90% after 10 years.9-12 Various preparation guidelines have been advocated for PLV; however, accuracy in the preparation of the margin is considered an important factor for success of dental restorations.13-17 The accuracy of the impression is affected by presence of blood in the area of the margin of the restoration.16 Unfortunately, the use of rotary instruments to refine the cervical margin of the preparation and the same impression technique can injure the periodontal ligament and provoke bleeding.18,19 New technology has been introduced to the market that reduce the risk of periodontal injury and bleeding. sonic handpiece, which was designed to be used with interchangeable diamond sonic tips (KOMET USA), developed i n conju nc2 tion with internationally renowned Italian specialist Dr. Domenico Massironi. The purpose of the following case report is to present the application of the KOMET SF1LM sonic handpiece and sonic tips for preparation of PLV restorations in a patient requiring esthetic rehabilitation. AT A GLANCE 1. Frontal view of the patient presented with a chief complaint of an unattractive smile. 2. Preoperative view of the anterior teeth. 3. The esthetic mock-up with composite material. 4. The diagnostic wax-up. 5. Facial view of the right anterior tooth after initial preparation. Note the supragingival ⇒nish-line placement. 6. Facial view of the left anterior teeth after initial preparation. Note the supragingival ⇒nish-line placement. 7. Facial view of the anterior teeth after ⇒nish-line placement. 8. Final impression of the preparation. 9. Postoperative facial view. Case report A 43-year-old woman presented with a chief complaint of an unattractive smile (Figs. 1 and 2). The medical history of the patient was non-contributory, and she was concerned with the appearance of her teeth because of some slight crowding and discoloration. Different options were offered to the patient, and risks and benefits of each option were discussed. The patient refused the option of orthodon- 4 tic alignment because of length of the treatment. She opted for porcelain laminate veneers. The purpose of the treatment was to address the patient’s chief complaint while preserving vital tooth structure. 3 Sonic boom Sonic instruments provide the precise removal of tooth hard tissue with preservation of gingival health.20 Currently, there are several sonic handpieces on the market such as KOMET USA’s SF1LM SF1LM SONIC HANDPIECE FEATURES Designed to assist in prepping tooth structure for crown placement Built-in halogen light Three different power settings, with spray cooling KOMET USA komet-usa.com 888-566-3887 92 DENTALPRODUCTSREPORT.COM | February 2011 http://www.komet-usa.com http://www.DENTALPRODUCTSREPORT.COM Table of Contents for the Digital Edition of Dental Products Report - February 2011 Dental Products Report - February 2011 Table of Contents From the Editor Readers’ Letters Special Report Clicks & Picks 10 Questions Product Watch DENTSPLY Caulk’s SureFil SDR Flow Demandforce D3 Keep Your Data Secure Ivoclar Vivadent’s Multilink Automix DENTSPLY Caulk’s TPH3 Clinical 360° The CAD/CAM Chorus Reducing Contamination in Your DUWLs.
